Invite at discharge

The discharge nurse helps the family install the mobile app; medications are entered and reminders start the same evening.

Physician link and consent

Your physician sends a connection request; the family sees exactly which data will be shared and approves. Consent is recorded.

Data flows from home

Family or caregiver enters blood pressure, pulse and daily status in the mobile app; data appears in the panel instantly.

Early intervention

A worsening patient is called to an outpatient check before ending up in the ER; the appointment is created on the web.

What does it cost our hospital?

The institution panel is billed monthly per tracked patient; there is no separate setup or training fee. Write to us through the contact form for a quote at your scale.

How long does setup take, and what is needed?

Our team creates the hospital record; no technical integration or HIS connection is required. Once the coordinator account is defined you simply invite your physicians — typical setup takes one business day.

How is patient data accessed?

Access depends on the patient's explicit consent and is scoped per institution: you see only patients who consented to your institution, and only the measurements they consented to. Consent is withdrawn the moment the patient does so from their phone.

What is your position under data protection law?

The institution is the data controller for its own patients' data; WellFamily acts as data processor in its capacity as infrastructure provider. A data processor undertaking is signed as an annex to the contract.

How do our physicians connect?

A physician opens their own account and selects your hospital; the membership is created inactive and does not appear on the hospital page until your coordinator approves it. This prevents unauthorised use of the institution's name.

How is our hospital listed in the directory?

Approved institutions are listed by province in the hospital directory; services, physicians, member discounts and patient reviews appear on your page. Discounts are shown only to signed-in users.
